Insurance & Charges
· Please bring your insurance card and a picture ID the day of surgery.
· We participate in Medicare, Medicaid, TRICARE & Workers Comp. To obtain information regarding current in network insurance plans, please call the center.
· We accept all insurances with out of network benefits and you will pay no more out of pocket than if you went to a facility in your insurance network.
· We will verify your insurance benefits before your surgery and give you a written estimate of fees you will be responsible for, including co-pay, coinsurance and/or deductibles. Remember, this is only an estimate.
· The Center’s bill does not include fees for your surgeon, anesthesiologist, laboratory tests, pathology tests or other diagnostic test ordered by your surgeon. These are billed separately by those providers.
Before Procedure
Please fill out the medical history form here. We will be calling you to review this with you.
· If you have a change in your physical condition such as a cold, fever or wound near your surgical site, notify your surgeon before coming to the surgery center.
· You will be receiving medications which make you comfortable during the procedure but which may remain in your body for some time. Arrange for a responsible adult to drive you home after your procedure and to remain with you the first 24 hours after your surgery.
· If you are on blood thinners, diabetic, high blood pressure or cardiac medications, check with your physician for instructions on taking these medications.
· You will be given instructions on when to discontinue food and liquids prior to surgery by one of our staff before the day of your surgery. Be sure to follow these instructions exactly as explained to you to avoid having your surgery cancelled.
A nurse from the center will contact you with preoperative instructions. If you have not spoken to a nurse from the center and it is the day prior to your procedure, please call 904-823-1447 between 8 a.m. and 5:00 p.m. If you have any questions and are unable to reach the center, please call your physician office.
Pre Operation form
Day of Surgery
· Bring a case for contact lenses, glasses, dentures or bridgework for safekeeping during your procedure.
· Bathe or shower and remove all makeup. Do not apply lotions or oil after bathing.
· Wear comfortable shoes and clothing with easy to button tops. Bring warm socks. Depending on your procedure, you may be able to leave some of your clothing on. Should your procedure require it, you will be provided with a patient gown and personal belonging bag for your clothing.
· A thorough preoperative assessment will occur upon admission. Please bring a list of current medications with dosages with you.
· Leave all valuables, including jewelry and cash, at home. The Center is not responsible for damaged or lost property.
· Anesthesia personnel will meet with you before surgery to discuss your anesthesia. They will explain to you what will happen before, during and after your procedure, and answer any questions you may have.
· One family member or friend will be allowed to accompany you before and after surgery into the patient care area. Have them dress warmly and/or bring a sweater or light jacket.
After Surgery
· You will rest in our recovery room under the care of our specialty-trained nurses. Your anesthesiologist will monitor and be updated on your condition while you are in the recovery room and until you are discharged.
· You will be given written discharge instructions. Because you have received medications during your procedure, the nurse will review the instructions with you and the responsible adult who came with you. These medications make you comfortable during the procedure, but they may remain in your body for some time. For your safety, we require that a responsible adult drive you home and remain with you after surgery or your procedure may be cancelled.
Post Operation form
Returning Home
· Even though you may feel fine, YOU MUST HAVE SOMEONE STAY OVERNIGHT WITH YOU.
· Do not drive, operate machinery, consume alcohol, or take any medication not prescribed by your physician.
· Call your surgeon if you have questions about what you can and cannot do, when you may return to work and when you can resume regular activities. These items will be discussed with you when you receive your post-operative instructions.
